After much research and looking around at things in my pond, I think my filter is a huge problem. How long should you keep lava rock or charcoal before you a. rinse it off or b. get rid of it completely and start new?

I do not know how much sludge you have but I would give Daves suggestion a try.

To better filter the water and prevent some of the sludge you need to get the solids into the filter. You need enough pump/circulation and or bottom contour to get the solids to the pump and some sort of chamber to collect the solids. Sediment tanks, vortexes, and radial flow all do this.
