and test and test and test the water to make sure there is nothing left of whatever caused your problems .Clean everything with peroxide and solar salt it and then treat city water .I have well water and i use a tote with a hole drilled in the side with a tank adapter with a plug for it and run the water through layers of quilt batting and the let it sit by the side of the pond until it comes up to temperature and then just unscrew the plug and let it go into the pond .Sometimes i use the pond filter to do it also

Zack, I'm sorry to hear your loss of a 4-year-old-koi. It's like losing an old buddy. I like to express my sorrow for you.

Prevention (i.e., covering up the pond) is a good way to avoid an accidental death of koi. Perhaps in the coming years you may want to cover up your pond to:

1) keep the leaves & dirts out,
2) keep water cleaner and from frozen over, and
3) avoid losing a koi during a Springtime massive clean-up operation.

To cover or not to cover? I've covered mine; I've saved myself some headache and my sanity :)
