Building onto an existing concrete patio the best thing to do is to put a couple rebar drilled into it a sticking up into the new block wall bring the rebar up just short of the height you want.
If your only going 24 inches than I'd say you don't need a rebar in every other bay drilled into the patio. But that is certainly the best and strongest way to go. Then once your walls are built rebar is in place then fill each bay with concrete. That will take a car hitting it and only crack it.
Once the wall is built place your liner or coating then top the wall off with a cap to your liking.
